Law Enforcement Video Training Catalog

Procedure for Loaning Videos and Receiving Credit

1. Fill out the AudioVisual Loan Agreement Form and send it to our office. We must receive this before requests for videos will be filled.
2. Mail or fax your request for videos using the Law Enforcement Video Request Form. Be sure to indicate the videos using numbers in this catalog.
3. Fill out Affidavit of Video Training Instructor
4. Complete Signup Sheet
5. Have each attendee complete green Record of Attendance sheets.
6. View videos and administer quiz.
7. Send the required paperwork back to us within ONE WEEK of the training session. The required paperwork includes: green sheets, affidavits, signup sheets and lesson plans (if choosing Training Option B-see "Curriculum" section for training options).
8. Return the videos to us via some traceable method (UPS, FED EX, etc).
9. Graduation certificates will be mailed to the video training instructor for distribution.

Affidavit of Video Training Instructor
The person administering the video training must fill out the Affidavit of Video Training Instructor. Each instructor should fill out this affidavit for each training session. By signing this affidavit, the video instructor agrees to also notify TAC when the video training is planned. You may notify us via fax or email. Once notified, TAC may or may not send a Law Enforcement Specialist to monitor your video training session. The affidavit also tells us whether your agency has decided to choose Training Option A or B (see Curriculum section).

Signup Sheets
Signup sheets must be filled out at every video training session. If more than one video is shown at a training session, there is no need to fill out two different signup sheets. Simply put each title of the videos shown on the signup sheet and advise the participants that they should put down one hour for each video on their Green Record of Attendance Sheet. Please note you must fill out a signup sheet for EVERY training session. We must have the printed name and signature of every person attending the training session.

Green Record of Attendance Sheets
Each attendee, who wishes to receive TCLEOSE hours for the time spent in training, MUST fill out a green Record of Attendance Sheet. We must have all of the information requested on the form, which includes name and social security number. Without this form, we will not be able to award TCLEOSE credit. Please be aware that TAC must receive this paperwork within one week of your training session. There is a two week return policy on videos unless additional days have been requested. Extensions will be granted unless another request for the video has been made. Always call to request additional days and please note: no matter how long videos are kept, we still need the required paper work within one week of your training session.
