Overview
Missed workdays due to treatment and maintenance of asthma, diabetes, heart disease, and cancer are all underlying factors that continue to drive up healthcare costs.  It is vital for employers to be proactive in keeping their employees healthy. 

That's why Texas Association of Counties Health and Employee Benefits Pool (TAC HEBP) has a more robust disease management program to address cancer, coronary artery disease, congestive heart failure, asthma, and diabetes. While only a small percentage of the population will be diagnosed with one of these conditions, that's where the majority of our healthcare dollars are going. When we can help these folks get a little healthier, we can substantially reduce overall costs.

TAC HEBP has partnered with Matria to deliver a cutting edge disease management program.  In 1994, a fundamental rethinking of healthcare services and management was already underway at Matria with the development of the first nurse-guided, education-oriented chronic condition management.  The program's clinical and economic outcomes were proven to employers and health plans nationwide, and disease management—accepted by many experts as the most successful way to improve the quality and cost-effectiveness of health services—was born.
