Care and Support Empowers Participants to Better Health
Blue Care Connection programs for Asthma, Coronary Artery Disease, Congestive Heart Failure, Diabetes, Low back pain and a wide range of cancers maintain an ongoing focus on the total health of each participant: advising, educating and supporting a healthier, more productive person. Condition management participants have access to a wide variety of services that provide the right help at the right time in the right way including:

  • Individualized telephonic coaching and education from registered nurses who speak one-on-one with participants to listen and advise on symptoms, medication, understanding their doctor's plan, setting healthy lifestyle goals and supporting their readiness to change.
  • NurseLine - a toll-free 24 hour support line where participants can talk with a registered nurse any time they have a health concern—big or small.


Good Health = Good Sense

It is hard to focus on your job when you are worried about a health problem. We all know someone who comes to work, but has their mind somewhere else. There's even a word for it now: "presenteeism," and it costs employers as much or more than absenteeism. The value of the disease management program through direct health care cost savings as well as through indirect gains in productivity as behavioral style changes begin to positively impact absenteeism and presenteeism rates. 

The bottom line is that healthier employees are crucial to lowering healthcare costs, reducing missed work days and improving productivity.

When you consider the fact that 50 to 70 percent of the dollars spent on health care are due to the lifestyle-related illnesses, counties can't afford not to worry about the health of their employees.
