Workers' Compensation Payroll Reporting System Policy/Member Conduct Form

Read the Payroll Information Submission Agreement below. Be sure you understand the Member Conduct Form in its entirety before using the system. If you have any questions or would like more information, call James Harvey at 800-456-5974.

You have elected to submit your payroll information to us electronically through the TAC web site. TAC is pleased that this option has proven to be convenient for you. However, before you can submit your information, we have to explain the terms of providing this service and obtain your agreement. If you register and submit your information, this constitutes a statement that you have read these terms and agree to them.

First, this and all TAC services are subject to the general Terms of Service, which can be reviewed at our Home Page.

In consideration of your use of this particular Service, you agree to: (a) provide true, accurate, current and complete information about yourself as prompted by the Service's registration form (such information being the "Registration Data") and (b) maintain and promptly update the Registration Data to keep it true, accurate, current and complete. If you provide any information that is untrue, inaccurate, not current or incomplete, TAC has the right to suspend all current or future use of the Service (or any portion thereof).

TAC PRIVACY POLICY 
Registration Data and certain other information about you is subject to our Privacy Policy. For more information, see our full privacy policy at www.county.org/abouttac/p_policy.asp.

MEMBER ACCOUNT, PASSWORD AND SECURITY 
You will establish your own username and password upon completing the registration process. Only you will control who has access to this information. You are responsible for maintaining the confidentiality of the password and account and are fully responsible for all activities that occur under your password or account. You understand and agree that TAC is not responsible for any damages resulting from the unauthorized use of your account or password. You further agree to (a) immediately notify TAC of any unauthorized use of your password or account or any other breach of security, and (b) ensure that you exit from your account at the end of each session.
 
Note: You are creating a username and password for the member county. Any authorized representative of the county can request this information. Please do not create a username or password consisting of personal information that you do not want shared with fellow employees.
