May the county judge refuse to put an item on the agenda?

No. Each commissioner has the right to place items on the agenda. Even if the court has ceded the administrative duty to prepare the agenda to the county judge, the actual authority over the agenda remains with the court as a whole entity. For more information on commissioners court meetings, see our new publication Commissioners Court Meetings Procedure and Decorum.
